Your privacy
Your health information is personal, and Jerri treats it that way. This statement explains what information she collects, how it’s used, and how it’s protected.
What we collect
- What you send through the contact form: your name, email, phone number, preferred way to meet, and your message.
- What you share during coaching, such as your health goals, history, and any notes Jerri takes to support your progress.
- Payment details, which PayPal handles. Jerri sees that you paid, but she never sees your full card number.
- Insurance details, if insurance billing becomes available and you ask her to bill your insurance.
How it’s used
Your information is used only to respond to you, schedule and provide your coaching, process payments, and, if insurance billing becomes available and you ask her to use it, bill your insurance. It is never sold or rented, and it isn’t used for advertising.
Who sees it
Only Jerri. She shares information with your doctors or other members of your care team only when you ask her to or give permission. The exceptions are sharing with your insurance company for billing you’ve requested, and cases where the law requires it, such as a threat to someone’s safety.
How it’s protected
The website uses a secure, encrypted connection. Client notes and records are kept private and are only accessible to Jerri. Online sessions happen over Zoom (or another platform you choose). Email and text are convenient but not fully secure, so please use phone or a session for anything especially sensitive.
Your choices
You can ask to see the information Jerri has about you, ask her to correct it, or ask her to delete it when coaching ends, unless the law requires her to keep certain records.
